## Clearer policy coverage for Codex

* Codex tools missing from their declared catalog now receive an explicit **Not evaluated** result.
* These calls continue without policy coverage until the tools are declared.
* Use `rippletide codex catalog` to refresh the declared tool catalog.

## Clearer policy outcomes in Traces

* Traces now distinguish undetermined policy outcomes from failures, including when trace content is redacted.
* Successful tool execution remains separate from approval by a Rule.

**Breaking change:** The standalone Coding Agent APIs and the legacy `/code` dashboard have been retired. Contact Rippletide for migration guidance if your integration relied on these endpoints.
